Psoriasis is a long-term (chronic) skin problem that causes skin cells to grow too quickly, resulting in thick, white, silvery, or red patches of skin.


Normally, skin cells grow gradually and flake off about every 4 weeks. New skin cells grow to replace the outer layers of the skin as they shed.


But in Psoriasis, new skin cells move rapidly to the surface of the skin in days rather than weeks. They build up and form thick patches called plaques . They most often appear on the knees, elbows, scalp, hands, feet, or lower back.


It is non contagious.


Pathology of Psoriasis-


Psoriasis is an immune-mediated (or autoimmune) disorder that occurs when immune cells known as T lymphocytes, or T cells, attack healthy skin cells in both the nonvascular horny outer layer of the skin and its deeper vascular layer. This attack causes the life span of the skin cells to shorten to about 3 to 5 days (skin cells normally live about 20 to 28 days) and forces the cells to reproduce more rapidly than normal.


Causes of Psoriasis-


The exact cause remains unknown. There may be a combination of factors, including genetic predisposition and environmental factors. It is common for psoriasis to be found in members of the same family. The immune system is thought to play a major role.


Triggers:


  • Infections, such as strep throat or thrush

  • Injury to the skin, such as a cut or scrape, bug bite, or a severe sunburn

  • Stress

  • Cold weather

  • Smoking

  • Heavy alcohol consumption

  • Certain medications — including lithium, which is prescribed for bipolar disorder; high blood pressure medications such as beta blockers; antimalarial drugs; and iodides.

Types of Psoriasis-


Scalp Psoriasis


This type involves plaques on the scalp that typically can be confused as dandruff. About half of all people with any type of psoriasis also have scalp psoriasis.


Plaque Psoriasis


Found anywhere on the body, plaque psoriasis is the most common form of psoriasis. Small red bumps spread and dead skin cells easily flake from those areas.


Inverse Psoriasis


This type of psoriasis involves smooth inflamed lesions in areas where the skin folds or flexes, such as the armpits, groin, or under the breast.


Erythrodermic Psoriasis


This kind causes severe disruption to the body’s chemical balance, affecting the majority of the body, and causes symptoms such as severe scaling, pain, and itching to the point where the skin looks as though it has been burned. It can also cause severe illness.


Pustular Psoriasis

A type of pustular psoriasis, it causes pustules to form at the sides of the heel or base of the thumb that turn brown and peel.


Guttate Psoriasis


This involves small, red dots that enlarge rapidly and often develop scales in the scalp, arms, legs, and torso. They can clear up without any treatment, but may appear later as plaque psoriasis. 


Nail Psoriasis


This form of psoriasis affects the fingernails and toenails with pitting, discoloration, flaking, or possible separation from the nail bed.

Psoriatic arthritis is a chronic disease characterized by a form of inflammation of the skin (psoriasis) and joints (inflammatory arthritis).


It features patchy, raised, red areas of skin inflammation with scaling. Psoriasis often affects the tips of the elbows and knees, the scalp and ears, the navel, and around the genital areas or anus.


The inflamed joints become painful and swollen and sometimes hot and red.  Joint inflammation in the fingers or toes can cause swelling of the entire digit, giving them the appearance of a cocktail sausage.


Joint stiffness is common and is typically worse early in the morning.


Psoriatic arthritis can also cause inflammation of the spine (spondylitis) and the sacrum, causing pain and stiffness in the low back, buttocks, neck, and upper back. In about 50% of those with spondylitis, the genetic marker HLA-B27 can be found.


Patients with psoriatic arthritis can also develop inflammation of the tendons (tendinitis) and around cartilage. This inflammation may lead to an enthesopathy, which is inflammation of a tendon at the site where it inserts into the bone. Inflammation of the tendon behind the heel causes Achilles tendinitis, leading to pain with walking and climbing stairs. Inflammation of the chest wall and of the cartilage that links the ribs to the breastbone (sternum) can cause chest pain, as seen in costochondritis.


The onset of psoriatic arthritis generally occurs in the fourth and fifth decades of life. Males and females are affected equally.


Causes of Psoriatic Arthritis-


The cause of psoriatic arthritis is currently unknown.


A combination of genetic, immune, and environmental factors is likely involved. In patients with psoriatic arthritis who have arthritis of the spine, a gene marker named HLA-B27 is found in about 50% of cases. Blood testing is available to test for the HLA-B27 gene. Several other genes have also been found to be more common in patients with psoriatic arthritis. Certain changes in the immune system may also be important in the development of psoriatic arthritis.


Types of Psoriatic Arthritis-


  1. Symmetric psoriatic arthritis: Symmetric arthritis affects the same joints — usually in multiple matching pairs — on opposite sides of the body. Symmetric psoriatic arthritis can be disabling, causing varying degrees of progressive, destructive disease and loss of function in 50% of people with this type of arthritis. Symmetric psoriatic arthritis resembles rheumatoid arthritis.

  2. Asymmetric psoriatic arthritis: Asymmetric arthritis typically involves one to three joints in the body — large or small — such as the knee, hip, or one or several fingers. Asymmetric psoriatic arthritis does not affect matching pairs of joints on opposite sides of the body.

  3. Distal interphalangeal predominant (DIP): Distal interphalangeal predominant psoriatic arthritis involves primarily the small joints in the fingers and toes closest to the nail.

  4. Spondylitis : Spondylitis affects the spinal column and may cause inflammation and stiffness in the neck, lower back, spinal vertebrae, or sacroiliac region (pelvic area), making motion difficult. Spondylitis also can attack connective tissue, such as ligaments, or cause arthritic disease in the joints of the arms, hips, legs, or feet.

  5. Arthritis mutilans: Arthritis mutilans is a severe, deforming, and destructive form of psoriatic arthritis that primarily affects the small joints in the fingers and toes closest to the nail. This leads to lost function of the involved joints. It also is frequently associated with lower back and neck pain. Fortunately, this type of psoriatic arthritis is rare. 

Treatment of Psoriasis-


Generally, the treatment of arthritis in psoriatic arthritis involves a combination of anti-inflammatory medications (NSAIDs) and exercise. If progressive inflammation and joint destruction occur despite NSAIDs treatment, more potent medications such as methotrexate (Rheumatrex, Trexall), corticosteroids, and antimalarial medications (such as hydroxychloroquine, or Plaquenil) are used.


Nonsteroidal anti-inflammatory drugs such as aspirin, indomethacin (Indocin), tolmetin.

(8)Sulphur — for skin disorders that are accompanied by intense itching. This remedy is most appropriate for individuals who are thirsty, irritable when not feeling well, uninspired and messy under ordinary circumstances, and who describe a sensation of internal heat and burning. Symptoms tend to improve with open, cold air and worsen with warmth.


(9)Kali Arsenicosum- Kali Arsenicosum has marked action on skin disorders like eczema, psoriasis and ulcers. There is dry, scaly eruptions with itching worse from warmth, walking and undressing. It is also useful in chronic eczema. There are fissures in bends of arms and knees. There are numerous small nodules under the skin. Intolerable itching worse while undressing in psoriasis.


(10)Kali Sulphuricum- Kali Sulphuricum has marked action on psoriasis. There are burning, itching, papular eruptions. Skin is dry, hot and burning. Itchy eruptions in nettle rash in children. Kali Sulph also helps in seborrhoea or dandruff and ringworm of scalp or beard with abundant scales. Sores on skin with thick, profuse, yellow watery secretion.
